TO FILE A CLAIM
1. If you have an accident or illness on
a Trip, call the assistance company at the
toll-free number shown on your ID card attached
to the AIG International Services (AIG Assist)
brochure. AIG International Services provides
the travel services outlined in Part 3 of
this Brochure, including assistance with
claim forms. AIG International Services
will NOT be able to verify your eligibility
until AFTER they have begun providing the
assistance services you need.
2. To obtain a claim form, contact The Claims
Administrator, American International Companies
Accident & Health Claims, P.O. Box 15701,
Wilmington, DE 19850-5701, toll free at
1-800-551-0824 from within the US. or Canada,
or from outside the U.S. or Canada call
collect at 1-302-761-3700.
3. To confirm your eligibility for insurance
benefits or assistance services under this
Plan, or to obtain a replacement ID Card,
call the Participating Organization at 303-988-9626
or 800-777-7665.
